Notice is hereby provided that an Application concerning the employment of one or more alien workers for the following permanent position will be filed with the U.S. Department of Labor. This Notice of Filing is provided in compliance with 20 CFR 656.10 (d) and will be posted for 10 consecutive business days, ending between 30 and 180 days prior to filing the permanent labor certification application. POSITION TITLE: RN CASE MANAGER
POSITION DUTIES:
Registered Nurses (RN) to work in an interdisciplinary team under My Choice Wisconsin Family Care Program focused on Long Term Care Services for eligible members.
Responsibility includes, but are not limited to, coordination of care and management of member's ongoing health and safety outcomes.
The RN Case Manager is responsible for the completion of the initial and comprehensive assessment and completion of the Member Centered Plan according to standardized procedures.
The RN will establish and monitor wellness and health promotion activities based on the member's assessment and identified personal and clinical outcomes.
POSITION SCHEDULE/SHIFT: Immediate first shift openings from Monday through Friday with flexibility, normally from 9am to 5pm. No weekends or holidays.

MINIMUM POSITION REQUIREMENTS:
RN shall have current licensure to practice in Wisconsin.
RATE OF PAY: From $60,000 per year, depending upon experience & education. The Employer will pay or exceed the prevailing wage, as determined by the U.S. Dept. of Labor.
