Our Process

Admission Procedure

Step 1:

Assessment

An RN supervisor will visit your home and do an assessment of your needs

Step 2:

Plan of Care

A plan of care will be disigned in coordination with your family and your physician

Step 3:

Support

Continuous outgoing supervision and monitoring of services provided

Services Provided

Skilled Nursing

An individualized professional nursing care service to clients under a physician's prescribed plan of care

Home Health / Personalized Care Aid

Non-professional personal person care services to clients under the supervision of a registered nurse.

Homemaking

Housekeeping and home management activities (cleaning, shopping, meal preparation, laundry, etc.)

Nutrition Consulting

Instructional guidance of client / family in normal and special dietary recommendations with shopping assistance

Physical Therapy

Rehabilitation exercises and treatments for clients with disabling conditions (under a physician's orders)

Occupational Therapy

Supportive activities to assist clients in their self-care activities of living (under a physician's order)

Speech Therapy

Speech instruction and treatment (under a physician's order)

Referrals

Request for services will be accepted from agencies, hospitals, clients' families, friends and other appropriate sources. If you are elderly, disabled, chronically ill, or recovering from a short-term illness and need assistance with household tasks or nursing services in order to remain in your home and live independently, you can benefit from a variety of home care services provided by Metro.

Respite Care

In some instances, assistance and relief are provided to individuals who are caring for elderly and disabled relatives in the home. Call for an assessment of the client's / family's needs.
